The standard choice form is issued by the Australian Taxation Office (ATO) for use by both employers and employees.
Use the form to advise an employer of your choice of super fund. You need to provide the information requested so your employer can make contributions to your nominated super fund.
Your employer will fill out Section B, which tells you the fund your employer will contribute to if you do not choose a fund.

Use the form to offer eligible employees their choice of super fund. You must fill in the details of your nominated super fund, also known as your default fund, before giving the form to an employee.
GESB administers the nominated funds for many WA public sector agencies: GESB Super (open to new members) and West State Super (closed to new members). These public sector schemes are deemed to comply with MySuper requirements.
